center established in 1983 to provide counseling and psychotherapy for
adults, children and families. |

| Who can use Counseling Care
and Services? |
|
| It is open all in need. There are no
limits as to the area that we serve. The professional staff, however,
reserves the right to refer people elsewhere if other services would better
meet the needs of their clients. |

| What are the costs for the
services? |
|
| Fees are based on a sliding scale
ranging from $20 to $65 for each visit. some insurances are accepted.
programs are offered at a predetermined fee. Presently, we do not accept
Medicaid, but we do attempt to establish fees affordable to everyone. |

| How can services be obtained? |
|
| Any individual in need of help can
refer himself to the agency by phone, in person or in writing. We also
receive referrals from schools, physicians, social services agencies or
other community helpers. |

| What about confidentiality? |
|
| We handle all clients information with
strict confidentiality. The informed written consent of the client is
required to release any information. We are compliant with the HIPAA
(Health Insurance Portability and Accountability Act of 1996) |

| When is Counseling Care &
Services open? |
|
| Monday through Friday from 9:00 to
5:00 pm and weekday evenings by appointment. |
